New Enrollment - My child has NEVER attended or been registered in Mesa Public Schools (PK-12)
Returning – My child is new to Taylor, was previously enrolled in Mesa Public Schools, but is not currently enrolled in Mesa Public Schools (inactive) - you will need to know your parent portal information
OLR (Online Re-enrollment) – My child attended Mesa Public Schools last year (existing/staying) - I am confirming student information

Updated Shot Records (Immunization Records)
Proof of Address (Bills, Lease, mortgage, etc)
Parent ID
If applicable - Withdrawal form from previous school
If applicable - Copy of current grades
If applicable - IEP, MET, 504 documentation(we cannot begin providing special education services until we have received a current copy)
If applicable - Court documents (custody or guardianship)
